Children With Disabilities(CWD)

The Children With Disabilities Service is a Specialist service which supports children who have profound, severe, complex and long-term needs, and their families. It provides access to a wide range of support for children and young people according to assessed need from age 0 to 19 years, and then on until they engage with Adult Services.
The team deals as a social work team with Children in Need, Children on Child Protection Plans and Looked After Children as well as Care Leavers.
The service is accessed by referral through the Referral and Assessment Service (Link R&A Service), usually by one or more of the local services with whom the child or young person and their families are already engaged.
On referral to the R& A service there is a careful assessment of the needs of the child or young person and their family in partnership with them. This identifies which care service(s) is (are) best placed to help them. If the referral continues to CWD a further assessment is carried out to ensure that support or intervention is appropriate and at the right level to support the family and the child or young person. CWD then draws down that support.

Lambeth short breaks policy
We are currently in the process of retendering our short breaks service within Lambeth. Our Commissioning and Children with Disabilities team are working together to ensure that the new short breaks statement is fit for purpose, whilst also linking it to the new service set-up. The new short breaks policy will be finalised by September 2017.
